To view, edit, or add a form, navigate to settings. Under the “Forms” category, select the form you would like to work on.
Forms include standard fields, which can be marked as included or not included, and required or not required. You can also choose to receive a notification on the Dashboard if a customer edits their answer.
Forms can also include custom questions. Custom questions let you ask anything you want and allow you to choose the type of response, like a yes or no answer, a text box, or a checkbox. You can also decide if feeding or medication instructions should appear on the booking sheet should a customer answer a specific question. Lastly, you can add conditional questions, meaning they only appear based on previous answers.
To add a conditional question, follow these steps:
- Go to “Add Your Own” and select “Add Custom Field.”
- Type your question in the textbox at the top of the page, labeled “Form Field Question.”
- Under “How will the user respond,” select “Yes or No.”
- To create your follow-up question, repeat steps 1-3.
- Choose the desired response type under “How will the user respond.”
- Select “Yes” for “Is this question conditional?”
- Set the conditions by specifying the previous question and answer required to prompt this new question.
